How Can Heat Therapy Help Relieve Fibromyalgia Pain? New Research Reveals Promising Mechanisms

Study Design

In this pilot study, 8 women with fibromyalgia completed a 4-week hot water immersion heat therapy program. Participants attended three sessions per week, spending 45 minutes immersed in 40.5°C water, with core body temperature maintained between 38.5°C and 39.0°C.

Researchers evaluated pain intensity, fibromyalgia impact, physical function, sleep quality, inflammatory biomarkers, and Heat Shock Proteins (HSPs) before and after the intervention.

 

What Did They Find?

After four weeks, participants showed significant reductions in both average and worst pain scores, together with improvements in fibromyalgia impact, physical function, and sleep-related impairment. These findings suggest that repeated heat therapy may effectively reduce symptom burden without additional pharmacological treatment.

Beyond the clinical improvements, researchers identified important biological adaptations associated with repeated heat exposure. Heat therapy significantly reduced HSP90, a protein linked to inflammatory signaling, while increasing HSP40 and HSC70, proteins involved in cellular protection and stress resilience. The number of circulating extracellular vesicles also increased, suggesting enhanced systemic transport of protective cellular signals.

Although inflammatory cytokines did not change significantly, the authors propose that modulation of Heat Shock Proteins may represent one of the primary mechanisms underlying the analgesic effects of heat therapy.

 

 

Why It Matters

Fibromyalgia remains a challenging chronic pain condition with limited long-term treatment options. This study suggests that heat therapy may do more than provide temporary warmth—it may trigger biological adaptations that support cellular resilience and the body’s natural pain-regulating mechanisms.

While larger randomized clinical trials are still needed, these findings strengthen the growing evidence supporting heat therapy as a valuable complementary approach for chronic pain management and highlight Heat Shock Protein modulation as a promising mechanism of action.
